What to bring
- Government-issued photo ID (driver's license or state ID).
- The name and contact info of your DER (Designated Employer Representative) or HR contact.
- A copy of your positive test result, refusal letter, or violation notice, if you have one.
- Any prior treatment records or SAP paperwork, if applicable.
- A written list of current medications, including dosages.
What to expect
The evaluation is a live conversation conducted virtually by secure telehealth. It usually takes 60 to 90 minutes and covers your history, the events that led to the violation, and your current situation. There are no trick questions — the goal is an accurate clinical picture so your SAP can recommend the right next step.
How to prepare mentally
- Be honest. The SAP is legally and ethically required to keep your information confidential and share only what is authorized. Honest answers lead to accurate recommendations and a smoother process.
- Take it seriously, not personally. The SAP process is federal — everyone with a violation goes through it. You are not being judged.
- Ask questions. You will leave with a clearer picture of what happens next, what your recommended program looks like, and how follow-up testing works.
Setting up telehealth
All appointments are virtual. To prepare:
- Choose a private, quiet room where you will not be interrupted.
- Use a device with a working camera and microphone.
- Test your internet connection ahead of time.
- Have your ID ready to show on camera.
After the evaluation
Your SAP will send a written recommendation to your DER — typically within a few business days. You then enroll in the recommended education or treatment program. Once that is complete, you come back for a virtual follow-up evaluation, and your employer schedules your return-to-duty test.
