How Long Does the DOT SAP Process Take?

Most drivers can complete the required steps in a few weeks — but the exact timeline depends on the treatment recommendation and how quickly each step is scheduled.

The realistic timeline

There is no fixed number of days set by DOT — the process moves at the pace of your evaluation, education or treatment, and testing. In practice, a driver who starts right away and follows every recommendation typically returns to duty in 3 to 6 weeksfor education-only cases, and 6 to 12 weeks or more when treatment is recommended.

Step-by-step time breakdown

1. Initial SAP evaluation — often within days

At Bailey's, we usually schedule your first appointment within a few business days of your call. The evaluation itself is a single 60–90 minute session.

2. Education or treatment — the biggest variable

  • Education programs can run from a few sessions to a few weeks.
  • Outpatient treatment typically runs 4 to 12 weeks.
  • Intensive outpatient (IOP) or inpatient can extend the timeline further.

Your SAP recommends what is clinically appropriate — not the shortest or longest option.

3. Follow-up SAP evaluation — 1 session

Once you have completed the recommended program, you return to your SAP for a face-to-face follow-up evaluation to confirm compliance and readiness.

4. Return-to-duty test — scheduled by your employer

After the follow-up, your SAP sends a report to your employer's DER. The employer then arranges a directly observed return-to-duty test. A negative result clears you to return to safety-sensitive duty.

5. Follow-up testing plan — 12 months to 5 years

Your SAP sets a schedule of unannounced tests: at least 6 tests in the first 12 months, with additional testing possible for up to 5 years. These tests happen while you are working — they do not delay your return to duty.
