This notice describes how medical information about you may be used and disclosed by Bailey's Counseling Service, PC and how you can gain access to this information. Please review it carefully.

Uses & disclosures

Protected health information (PHI) may be used for treatment, payment, and healthcare operations, and disclosed as required or permitted by law. DOT SAP reports are disclosed to your Designated Employer Representative as required by 49 CFR Part 40.

Your rights

You have the right to inspect and copy your records, request amendments, request an accounting of disclosures, request confidential communications, and file a complaint. Requests should be made in writing.
