Required
- Government-issued photo ID (driver's license, state ID, or passport).
- Employer / DER contact information: name, phone, email, and company.
Strongly recommended
- A copy of your positive test result, refusal notice, or violation letter.
- Any Clearinghouse notice you received.
- A written list of current medications, including dosages and prescribers.
Bring if you have them
- Prior treatment or program discharge summaries.
- Previous SAP paperwork (if you have been through the process before).
- Court documents related to the violation, if applicable.
- Insurance information for any recommended treatment program (SAP fees are self-pay, but treatment often is not).
For telehealth evaluations
- Have your ID ready to hold up to the camera.
- Scan or photograph documents in advance so you can email or upload them.
- Use a device you can move to show the room is private, if asked.
What if I don't have my paperwork?
Do not delay scheduling. We can start the evaluation with what you have and request the rest from your employer or MRO afterward.
