The return-to-duty test

The return-to-duty (RTD) test is a single, one-time test taken after your SAP has completed the follow-up evaluation and sent a report to your employer's DER. It must be directly observed, and the result must be negative before you can perform any safety-sensitive function.

• Ordered and scheduled by the employer, not the SAP.
• Directly observed (a same-gender collector watches the specimen leave the body).
• A single event — pass it, and you are cleared to return to duty.

Follow-up testing

Follow-up testing is a plan of unannounced tests that begins after you return to safety-sensitive duty. It is set by your SAP as part of your follow-up evaluation and stays with you across employers.

• Minimum: 6 tests in the first 12 months back on duty.
• Maximum: the plan can extend for up to 5 years.
• All follow-up tests are directly observed.
• Your SAP sets the number and type; your employer administers them on an unannounced schedule.
• The plan transfers with you — a new DOT employer must continue the schedule.

Quick comparison

• Timing: RTD happens before you return. Follow-up happens after.
• Frequency: RTD is one test. Follow-up is many, over months or years.
• Who sets it: Employer schedules RTD. SAP sets the follow-up schedule.
• Observation: Both are directly observed.
• Consequence of a positive or missed test: either restarts the entire SAP process.

What drivers often get wrong

• Assuming the RTD test ends everything. It does not — follow-up testing is still ahead.
• Thinking a new employer starts the follow-up count over. It does not — the existing plan continues.
• Missing a follow-up test because it is unannounced. Missed tests are treated as refusals.
